I counted 14 times on 1st, 2nd and 3rd down that we had 12+ yards to go. According to NFL.com we had 68 rushing or passing plays.

If my math is right, that's 20% of the time we are playing from way in the hole.

How are we winning any games at this rate?

After halftime the announcers, or Pam Oliver, quoted MM about the penalties on first downs. Supposedly MM said some to this effect;

"I've run out of first and 15 plays."

That's what's killing the offense. Two things really. You identified one, bad down/distance due to penalties and sacks. The other reason is the field position battle.
